Database access component fails silently

I must have something misconfigured in my local environment. I can't see any VMs, and when I try to create one, nothing happens.

SOME error must be happening that is not being reported. Not on the frontend, and not in the console.

Start by reproducing the missing-VM and VM-creation behavior in a local environment, then trace the database access component and the paths serving the frontend and console. Identify where errors are swallowed and determine what observable error reporting is expected for both operations. Done means failures are reported rather than silently ignored, with tests covering the affected paths.
