When I was debugging plan mode, I was able to reproduce the issue you mentioned. It seems that during plan mode, the propagation of the permissions mode it's in might be missing somewhere. I need to investigate more about that and fix it.
In terms of further DX improvements for plan mode, I am adding the following
- Make it easy to browse the whole plan when Ctrl + O is pressed: The plan is also not entirely readable in the TUI
- CMD will show that it's writing the plan but doesn't. Tracking that #1605
- Better permissions mode awareness is needed: #1601
