Agents crashs and stops without completing task some times, does not properly undertands folder taging

I have given a complete instruction for creating a database and its ingestion flow, but the flow worked only after running the database, which it failed a few times. Another time, even after tagging the full frontend to change the frontend, the instruction was very clear, but the website had no visible changes. It was not able to differentiate between changing path and just adding a query param. This issue happened across multiple sessions.
