Add "Blocked Website" category to the Report an issue page

### Problem Description

Currently, if an entire domain is blocked, you can only report it as a broken website. A broken website, where only a few elements are blocked but the page otherwise loads, should not be categeorized the same as a blocked domain.
### Proposed Solution

Adding a separate Blocked domain issue should make it easier to parse reports as in my experience, most blocked domains are a result of DNS filters and not content filters.

Additionally, if a domain is blocked, I shouldn't have to upload two pictures as one of them would be only a connection error.
