Add "Report incorrect blocking" item to Assistant menu

### Issue Details

Now the address must be manually copied and pasted into the report form. Sometimes users type the address manually and make typos.

Adding a new item to the Assistant menu will save users from unnecessary actions.

### Proposed solution

Add "Report incorrect blocking" item to Assistant menu.
For apps `package name` can be sent to the reporting tool.
