improve our error handling

we think our error handling could be improved
especially regarding file I/O code

Then inspect any nearby tests or logging/error utilities for the same flows and run the relevant suite before/after a focused change. Mark it done only when a specific subset of I/O error cases is defined, implemented, and verified.
