Code the patient profile dashboard screen in therapist app

This screen will appear after clicking on `view or manage patient`. This flow will have it's own bottom navigation bar as seen in the image. The PR should have the code for the dashboard including the bottom nav. Pls use icons from the design (figma file is in readme)
