Reject accounts with incomplete code

Consider blocking new account submissions which don't contain the full code of the account.

We intend to introduce a [code registry into the protocol](https://github.com/0xMiden/protocol/discussions/2982) which would enforce this. Until this feature is introduced, we should reject code which wouldn't let us reconstruct this registry as part of a migration.
