Add unit tests for middleware

Add configurable logging with sensible defaults.

First locate the middleware, model, and existing unit-test conventions in the repository. Reconcile the title's request for middleware tests with the body’s proposal for configurable logging and sensible defaults. Done means the intended logging behavior is implemented and covered by unit tests.
