Update middleware documentation

**Feature request**
It would be great to have timeout for hooks.

**Use case**
Many users need this functionality for production deployments.

**Implementation idea**
Add a new config option `enable_timeout` with default `false`.

Locate the middleware and hook execution entry points, then find how existing configuration options are parsed and documented. Clarify the timeout value, units, and behavior before implementation; done should include the `enable_timeout` option defaulting to `false`, documented middleware behavior, and coverage for the new setting.
