Add a section in [1] to explain the configuration when there is a cluster fronted by a LB
When there is a LB, add the LB DNS as the call back url.
This callback url is taken from the server.hostname in deployment.toml file. Therefore if its a APIM cluster then better to put the LB DNS under server.hostname
