Research direction

Start by locating MetadataKeyProviderName and the Token Based Rate Limiting usage shown in the issue, then trace how policy-engine data is passed through ctx.SharedContext.Metadata. Done means provider metadata no longer conflicts with policy metadata and the affected behavior is covered by tests; no test or source file is named in the issue.
