It's used in radical (where the offset is not mentioned but we align the top with the top of the radical bar) and for operator (where adjustment is needed see w3c/mathml#200, so the calculated ascent does not match the stretchy glyph ascent). I think it's easier to reason if the reference point is the top of the operator rather than its baseline.
