Describe the bug

Pinned icons occasionally go missing on the Windows 10 start menu, as shown below. The tile is still there and functional, but it is blank instead of showing the application's icon. In the example shown, the missing icon (circled in red) is for Visual Studio Code. Sometimes the Google Chrome icon goes missing; occasionally others do as well. I don't know what causes them to disappear, nor what causes some of them to come back eventually.

Image

I hear folks talking about the tile database or cache that Win11 doesn't directly interact with, so I guess that database or cache needs to be refreshed somehow.

Expected outcome

No missing icons

Actual outcome

Missing icons

Research direction

The report names no source file, test, or entry point. First reproduce the missing pinned icon on Windows 11 24H2 with ExplorerPatcher 26100.8457.70.1, checking Visual Studio Code and Google Chrome; done means pinned Windows 10 Start menu tiles consistently display their application icons.
