Description
With the Start-Menu in Win10 mode multiple extraneous PowerShell/Terminal entries appear.
Probably due to updates of Powershell.
Switching to Win11 start and back to Win10 solves this.

Research direction
Start by reproducing the duplicate PowerShell/Terminal entries in Windows 10 Start-menu mode using the screenshot and the reported switch to Windows 11 mode and back. Compare the Start-menu state before and after that switch; done means the Win10 menu no longer shows extraneous entries after PowerShell updates.
