Description
This page suffers from a non-obvious term selector, e.g. I may be wanting to edit a Fall Quarter Support Call but the page will load a different term for various reasons. Consider adopting the design of the Teaching Call Form with a term selector on the right sidebar. We could also adopt the checkbox on the right sidebar but turn it into an "at a glance / statistics" box, e.g. number of faculty called, percentage of call responded, etc.
