I'm on a mac and when I try to launch Codex, it seems to load some old codex home directory that doesn't reflect everything I've changed recently. I tries to load old MCPs that I've since removed and doesn't load new plugins that I've installed recently. How do I fix this?

Research direction

Start by reproducing the Codex launch behavior on macOS and inspect which home directory, plugins, and MCP configuration it uses. Compare the active configuration with the user's recently removed MCPs and newly installed plugins. Done means Codex loads the current plugins and no longer loads removed MCPs.
