[Bug] Codex not loading plugins, loading old MCPs

I'm on a mac and when I try to launch Codex, it seems to load some old codex home directory that doesn't reflect everything I've changed recently. I tries to load old MCPs that I've since removed and doesn't load new plugins that I've installed recently. How do I fix this?

Compare the active configuration with the user's recently removed MCPs and newly installed plugins. Done means Codex loads the current plugins and no longer loads removed MCPs.
