Infer, or configure, if the CSV file has a header?

Description
For now, we assume the CSV file will have a header.
We might infer it from some heuristic (not implemented in csv-range).
Or we might let the user configure it from the UI. I don't think it's a good idea though, as most of the CSV files have a header, and it would overload the UI.
