Description

Effectively the same as files-to-prompt but as a default fragment plugin.

Maybe even support this:

llm -f dir:my-project,py+md

Which would mean "every .py and .md file in the my-project/ directory".

Research direction

Start by reviewing how the existing files-to-prompt behavior works and how fragment plugins are defined and discovered. Compare the proposed dir:directory and dir:my-project,py+md forms with the current fragment syntax. Done means a default directory fragment plugin supports the requested directory and extension filtering behavior, with tests covering both examples.
