Update Wiki

Hi guys,

since the wiki is outdated and the help command within the cli as well claude-flow is pretty much unusable as soon as the happy path does not work. Could you please update the wiki and the inital readme files. For example the task entries are fully outdated in the wiki and/or most content is missing.
