Description

At the bottom of every page when a user is logged the footer contains "report a site issue".

Image

Currently the report a site issue goes to a typeform link, however, we lost the password (or something I'm not sure) and we can no longer get the responses from this form. We need to fix this.

Research direction

Start on the QA site and inspect the footer's “report a site issue” link, then review the existing Typeform URL and the completed pull requests listed in the issue. Replace the unavailable submission path with a Paperform that sends reports to casa@rubyforgood.org, refine the questions as needed, and verify the footer link and submission flow from the available user views.
