Annual Evaluations Form for actively-affiliated Organizations

first, add the evaluation form questions as part of the FormBuilder service and forms/new checkboxes

then add new code to the Form to detect Organization info so we don't need to re-ask on the Form what we already have in the db (same setup as the code that hides the Person Identifier q's if they're logged in).

Start by reading the FormBuilder service and the forms/new view, then compare the existing Form logic that hides Person Identifier questions for logged-in users. Identify the annual evaluation questions and the available Organization data before defining how the form should avoid repeated questions. Done means the questions are included and organization information is detected without re-asking for existing data.
