Description
In previous versions (18.1 was the last one I know worked to be exact) when the DTD defined in the DOCTYPE SYSTEM statement in the xml file was not available, the DTD file defined to be associated with the xml file via the xml.fileAssociations vscode setting was used as fallback instead, but in the latest version that's not the case anymore, e.g. I have to remove the DOCTYPE statement for autocomplete to work correctly.
Is is it possible to restore the previous functionality?

Research direction
Reproduce the case where a DOCTYPE SYSTEM DTD is unavailable while a fallback is configured through xml.fileAssociations. Trace the extension's DOCTYPE and file-association handling, then verify that XML autocomplete uses the associated DTD without removing the DOCTYPE statement.
