Research direction
Start with the call hierarchy documentation in docs/LSPSupport.md and trace how its Ctrl+Alt+H shortcut is registered alongside Find References. Reproduce the conflict with call hierarchy enabled, then verify that the Find References shortcut works again while call hierarchy remains available.
