Description

Originally reported as part of #2437 but this is a totally different issue that's not related to pipe first.
Minimal repro:

$ echo 'Array.set(arr,_,arg)' | ./_build/install/default/bin/refmt
arr[_] = arg;

Research direction

Start by reproducing the minimal example with the refmt executable at ./build/install/default/bin/refmt. Investigate how Array.set(arr,,arg) is handled when converted to indexing syntax, and consider the issue complete when this reported behavior produces the intended result.

Written by the indexing model from the issue text.
