Research direction
Start by reproducing the video-call audio behavior around RNCallKeep.getAudioRoutes() and RNCallKeep.setAudioRoute(uuid, route.name). Check why selecting the Speaker route still leaves output on the earpiece, and verify that a video call uses the loudspeaker when the route is selected.
