Research direction
Reproduce the behavior with a Quarto website configured with editor: visual, then use the Explorer to switch back to an already open Qmd file. The issue names no source files or tests, so first locate the Explorer and visual-mode navigation entry points. Done means selecting an open Qmd returns to its existing view without opening it again.
