Code annotation line highlight overflow and gutter miss-alignment

Bug description

The code annotation line highlights overflow on the left.
There is also a misalignment in the highlight between the line and the gutter (gutter is a bit lower).

Start by rendering the provided Quarto document with HTML format and code-annotations set to select, then inspect the generated annotation highlight and gutter alignment. Verify the fix against the reproduced case so the highlight no longer overflows on the left and the gutter aligns vertically with the highlighted line.
