callout title has no space between it and top callout border (pdf/latex)

Bug description

The title of a callout is not spaced appropriately for pdf output. The title basically has no padding.

Steps to reproduce

Expected behavior

I would expect at least a little space/padding between the title and the top border.

Actual behavior

There is no space/padding between the title and the top border.

This may be an issue with only the 'simple appearance'. Here is a shot of the results from setting callout-appearance: default:

Reproduce the provided .qmd callout example with PDF output and compare the simple and default callout appearances. Trace the generated LaTeX and callout styling to find where the title meets the top border; done means the title has visible padding in the simple appearance without regressing the default appearance.
