Research direction
Start in apps/server by tracing the create-new-worktree-per-chat flow and reproduce the issue by creating many chats on macOS. Identify where those worktrees are managed, then verify that completed or no-longer-needed worktrees are cleaned up and repeated chats no longer fill the disk.
