mem tracker needs more detail info in "slow query.log"

Some users say their mem trackers not working, however, we are not sure if the user makes a mistake in mem tracker's config. Additionally, we can't ensure whether the mem tracker's config is as expected.
If the slow query contains `quota` and `oom_action` of current mem tracker, it can help troubleshoot mem tracker problems.
