Description
Shortly before Christmas I imported the latest release of SNOMED. This breaks the builder for any codelists that contain ancestors of newly-introduced codes.
The fix is going to be quite involved.

Research direction
Start by reproducing the builder failure with a codelist containing ancestors of newly introduced SNOMED codes, using the latest SNOMED release described in the issue. Trace the builder's handling of those ancestors and verify that affected codelists build successfully without breaking existing codelist behavior.
