Description
Two related icon inconsistencies exist in the UI:
- A failing MCP shows more than one negative indicator at the same time (e.g. both a badge and an icon)
- The app uses two different green/red icon styles in different places — these should be unified into one consistent pattern
Acceptance Criteria
- A failing MCP shows exactly one negative indicator — no duplication
- All health/status indicators across the app use a single consistent icon style
