Description

Two related icon inconsistencies exist in the UI:

  1. A failing MCP shows more than one negative indicator at the same time (e.g. both a badge and an icon)
  2. The app uses two different green/red icon styles in different places — these should be unified into one consistent pattern

Acceptance Criteria

  • A failing MCP shows exactly one negative indicator — no duplication
  • All health/status indicators across the app use a single consistent icon style
