User avatar / profile image is not displayed in the header

Description
The icon at the top right looks like it should display the user's avatar, but it always displays a generic user profile icon.
See screenshot, showing my avatar correctly in the history log but not at all in the Studio header.
Note: this seems to be a broader issue, affecting the LMS too?
