I am in 'Ask for approval' mode but when I ask a question, codex regularly just makes changes. This is within a project and it's only making changes within that project but I would expect 'Ask for approval' to, well, ask for approval.
What steps can reproduce the bug?
Ask about code in a project, see it make changes without asking.
What is the expected behavior?
Ask for approval before making changes.
