Automatic approval fails with Guardian compaction checkpoint incompatibility

The permission UI shows “Ask for approval”, but the agent's effective configuration still reports approvals_reviewer=auto_review.

Before any manual approval card appears, operations fail with:

Automatic approval review failed: parent compaction checkpoint is incompatible with the Guardian review model or its compatibility is unknown

Restarting the app did not resolve the issue. File creation and messages to other tasks are blocked.

Expected behavior:
Show a manual approval card so I can approve a specific operation once.

What steps can reproduce the bug?

Observed in an existing long-running Codex macOS task:

1. Set the task permission UI to “Ask for approval”.
2. Ask the agent to perform an operation requiring approval.
3. The agent reports approvals_reviewer=auto_review.
4. Before a manual approval card appears, the operation fails with:

Automatic approval review failed: parent compaction checkpoint is incompatible with the Guardian review model or its compatibility is unknown

Restarting the app did not resolve the issue.
A minimal reproduction in a fresh task has not been established.

What is the expected behavior?

I need a working manual approval mode that displays an approval card and allows me to approve one specific operation using “Allow once”.

If the current settings or managed policy prevent this, the UI should clearly explain why and how to resolve it.
