Scoped filesystem permission is granted, but subsequent execution
intermittently fails before the shell starts:

Failed to create unified exec process:
helper_unknown_error: setup refresh had errors

Earlier CLI, TypeScript and ESLint checks succeeded.
Closing and reopening Codex has been attempted.

Ask for approval is enabled. Full access is OFF.

Please investigate and provide a supported fix without requiring
unrestricted access.
