Description

Subject: Ghost/deleted Project chat persists in macOS unified app Recents

Product: ChatGPT macOS (unified desktop app)
Version: 26.903.61454
Device: macOS on Apple Silicon

Issue:
A conversation that was part of a ChatGPT Project (created in “Chat” inside the Project) was deleted successfully server-side: it no longer appears on chatgpt.com or in the iOS app. However, the deleted conversation still appears as a “ghost” entry in the macOS app Recents list.

Ghost entry behavior (macOS app):

Clicking it shows: “Could not load this ChatGPT conversation”
Delete action returns an error
Archive action returns an error
Rename shows no error, but the displayed name does not change
The Project is pinned (the ghost chat itself is not pinned)
Server-side confirmation:
Opening the conversation in a browser shows: “Conversation has been deleted. Start a new chat.”

Troubleshooting performed:

Quit/relaunch ChatGPT macOS app
Sign out/in
Reinstall app
Reset default/local storage
Reset default/session storage
Reset codex-browser-app IndexedDB
Clean-profile test (moved ~/Library/Application Support/Codex and related cache dirs aside)
Unpinned/re-pinned the Project
Archived chats: ~110 archived; “Delete all” only deleted <10; ghost conversation ID not present in Archived Chats manager
Data export requested (pending)
Request:
Please investigate/fix stale Recents state in the macOS unified app that continues to surface the conversation even though the server reports it deleted.

Research direction

No source file or test is named. Start by reproducing the deleted-conversation state in the macOS unified app and tracing how the Recents list is populated and refreshed. Done means the deleted conversation no longer appears after relaunch, sign-in, or local-data reset, while valid Recents entries continue to work.
