Description
What version of the Codex App are you using (From “About Codex” dialog)?
26.903.61454
What subscription do you have?
Pro
What platform is your computer?
Windows 10 x64
What issue are you seeing?
Pull Request #6 (“Error with PayHit”) is incorrectly displayed under “Previously reviewed” in the Codex App.
My GitHub account does not have approval or write permissions for the repository, and I have not submitted a review or approval for this pull request. However, Codex lists the pull request under “Previously reviewed” and displays a reviewer avatar with a green status indicator.
GitHub currently shows the pull request as Open and not merged:
https://github.com/Zamirathe/Uconomy_Essentials/pull/6
Expected behavior:
- A user who has not submitted a review should not be marked as having reviewed the pull request.
- The reviewer field should only show accounts that actually submitted reviews.
- Codex should distinguish between viewing or opening a pull request and submitting a GitHub review.
Please check whether Codex is incorrectly interpreting local viewing history, synchronization activity, comments, or historical review data as a review performed by the current user.
What steps can reproduce the bug?
Feedback ID: 01a08776-7ca3-75c2-a67d-95722b607a17
What is the expected behavior?
The pull request should not appear under “Previously reviewed” unless the current user has actually submitted a GitHub review.
The reviewer field should only display accounts that have submitted reviews. Viewing, opening, synchronizing, or commenting on a pull request should not be interpreted as reviewing or approving it.
Additional information
This appears to be a mismatch between the Codex App review status and the actual GitHub review/permission status.
The pull request is currently Open and has not been merged. The issue was observed in the Codex desktop app on Windows 10 x64.
I have attached screenshots showing the Pull Request list and details view. Please verify whether local viewing history, synchronization data, or historical review records are being incorrectly mapped to the current user’s review status.

Research direction
Start by reproducing the issue with Feedback ID 01a08776-7ca3-75c2-a67d-95722b607a17 and Pull Request #6, then trace how Codex obtains GitHub review status and reviewer identity. Compare the app’s “Previously reviewed” result with the pull request’s actual GitHub reviews and permissions. Done means unreviewed users are excluded and viewing, synchronizing, or commenting does not count as submitting a review.
