What issue are you seeing?

My long-running conversation was stopped with a notice that, from memory, said it was stopped “as a precaution.” No actionable explanation was visible, and the notice is now gone.

Reopening the conversation only offers two options: resume a different chat or start a new one. I cannot send messages or run / feedback inside the affected conversation.

The local log records this error on September 9, 2026, at 11:05:16 a.m. Pacific (18:05:16 UTC): “This request was blocked by our safety systems. Reason: Potentially unintended activity.”

This conversation began July 19 and contains substantial accumulated scientific visualization context. Its saved local transcript still exists.

What steps can reproduce the bug?

1. Resume affected thread 019f7b77-bb62-7d31-97b7-156db922a9ef.
2. Encounter the prompt to resume a different chat or start a new one.
3. Select the resume option and select the affected conversation.
4. Return to the same prompt without being able to continue.

I do not know how to reproduce the original safety-system trigger. My last message was: “so would our underlying simulation approach basically be based on an effective susceptibility / atomic polarization moment for the Oxygen atoms?”

What is the expected behavior?

An explanation of what triggered the stop and a supported review/recovery path that preserves the original conversation’s accumulated context. If resumption is unavailable, clearly explain that instead of returning me to the same recovery prompt.

Additional information

I invoked /feedback from a separate recovery conversation because the affected conversation cannot accept commands. The generated issue URL referenced that separate conversation, 01a08779-d07f-75d2-9224-43268810a483, rather than the affected thread.

A subsequent local codex doctor diagnostic run completed with overallStatus “ok”; all checks passed.

Start by reproducing the behavior in codex-cli 0.153.4 on macOS with a long-running chat that stops as a precaution, then trace the chat reopening flow. Done means the stopped chat offers the intended resume or reopening path rather than only another chat or a new chat, with regression coverage for the behavior.
