Description
What version of the Codex App are you using (From “About Codex” dialog)?
26.901.51231 (Windows ChatGPT app, Powered by Codex & OWL; released Sep 5, 2026)
What subscription do you have?
ChatGPT Plus
What platform is your computer?
Microsoft Windows NT 10.0.26200.0 x64
What issue are you seeing?
Beginning September 7, 2026, Codex can access my MP4 video files but can no longer review them with picture and sound. This same workflow was functioning normally through September 6.
Codex repeatedly reports that the tools available in the session do not allow it to “watch and hear” the video. The files themselves remain accessible to Codex.
What steps can reproduce the bug?
Attach an MP4 containing video and microphone audio to a Codex task.
Ask Codex to review both the visual performance and audio.
Codex accesses the file but reports that the session's available tools do not allow it to watch and hear the video.
Reproduced across multiple fresh tasks, including after completely signing out and back in.
Also reproduced with two different versions of the media: the original OBS-recorded MP4 and a newly exported compound MP4 with audio incorporated into the new export. Codex also attempted to locate/use FFmpeg but remained unable to review picture and sound.
What is the expected behavior?
Codex should be able to use its available media/computational tools to inspect the MP4's visual content and audio as it did previously.
Additional information
This is a regression. The workflow worked normally through September 6, 2026 and first failed September 7, 2026.
I also submitted an in-app bug report with diagnostic logs.

Research direction
The issue names no repository files, tests, or entry points. Start by reproducing the MP4 review failure in the Windows Codex app and inspecting the diagnostic logs associated with feedback ID 01a07d59-0a2a-71b3-8212-9548e6c4491b. Done means Codex can again inspect both video and microphone audio from attached MP4 files.
