GPT-6: safety pause keeps returning seconds after clicking Continue

I'm using GPT-6 in the Codex desktop app on Windows (currently installed version: 26.901.6511.0). In an existing project conversation, I frequently get the “Chat paused as a precaution” dialog.
I review the findings, check the acknowledgement box, and click Continue. Within a few seconds, the dialog appears again with substantially the same explanation. Repeating the confirmation leads to the same pause → continue → pause loop, preventing normal progress in the conversation.
Is this a known issue with the continuation flow? How can I recover the affected conversation? If something still requires review, the dialog should explain what remains unresolved. Please let me know which logs or diagnostic IDs would help investigate.
