Suspected false positive: authorized defensive code review
Please investigate a Codex security-review request rejected for possible
cybersecurity risk during maintenance of our own Novact A2A repository.
- Repository: novact-ai/novact-os-a2a-poc, PR 30.
- Review run: 20260904T200230Z-6604-c0d847ea (September 4, 2026, UTC).
- Codex review thread ID: 01a06e05-4d77-7522-8d04-65b91d015b8a.
- Context: reviewing defensive mutation fencing, lease renewal, and process
identity checks, including regression tests, in an isolated source snapshot. - No external target testing was requested.
The terminal events were error and turn.failed, with this message:
This content was flagged for possible cybersecurity risk. If this seems wrong, try rephrasing your request. To get authorized for security work, join the Trusted Access for Cyber program: https://chatgpt.com/cyber
The gate recorded RUNTIME_ERROR, reason Codex CLI exited 1. The response
did not identify an offending code line or return a security finding. This
does not establish the underlying cause or whether access provisioning is
missing. Please assess the suspected false positive and advise the supported
resolution for this account and Codex CLI surface.
Our review template also incorrectly identifies this Novact repository as
EngageIQ and contains unrelated telephony policy text. We have identified
that review-context defect, but cannot determine whether it contributed
to the rejection.
Please advise whether a false-positive correction or specific Trusted Access
approval/provisioning is required before the required review can proceed.
No bypass of the provider decision or CI approval is requested.
This report intentionally excludes credentials and private source code.
It has been prepared locally and has not been submitted.

Start with feedback ID 01a06dee-1851-7420-9373-fe8cf4d65823, review run 20260904T200230Z-6604-c0d847ea, and the recorded error and turn.failed events. Compare the reported repository-context mismatch with the security rejection; done means documenting whether the cause is a false positive, missing Trusted Access provisioning, or a context defect, along with the supported resolution.
