What issue are you seeing?
User-reported incident: Codex was unavailable beginning September 3, 2026 and remained unavailable into September 4, 2026.
During the affected period, local Codex logs repeatedly showed sanitized symptoms including:
- Codex app-server is not available
- Remote environment is offline
- EPIPE connection failures
- Several configured remote hosts failed to resolve or reported offline
The Codex App later recovered and is currently able to run local tasks. Codex CLI installed alongside it is 0.154.0-alpha.3.
This report does not assert that the entire OpenAI platform was down. Please correlate account and backend telemetry for September 3-4 to determine whether this was a service-side incident, an app-server reconnection failure, or a client/remote-host interaction.
What steps can reproduce the bug?
- Open Codex Desktop on macOS.
- Attempt to open or run local and configured remote tasks.
- Tasks fail to connect or sync, with app-server unavailable and remote-offline errors.
- Retrying and reopening did not restore reliable availability during the reported window.
- The app later recovered without a clearly identified user-side remediation.
The failure is no longer consistently reproducible now that service has recovered.
What is the expected behavior?
Codex Desktop should start its app-server, sync the task catalog, and either connect to configured environments or clearly isolate an unavailable remote host without making the overall app unavailable.

Timing is user-reported in America/Chicago time: outage began September 3 and recovery was observed September 4, 2026.
