Research direction
Start by reproducing the Ctrl-d behavior described in the issue and determine whether it is captured inside or outside the Codex terminal app. Trace the Pets feature and its shortcut handling from the relevant entry points; done means users can disable or remove Pets and Ctrl-d no longer affects unrelated applications.
