Research direction
Start by reproducing the issue in Codex App 26.818.61809 on macOS Apple Silicon using the affected chats “Verschillen recente items” and “Cd importeren Windows 11.” Check whether the chats reappear in Recent after deletion or archiving and whether opening them returns a 404; done means affected chats stay removed and no longer produce these errors.
