What issue are you seeing?

codex left an uncompiled code when it was done coding. (first mistake)
then when asked if the issue is related to this session - it found no problem (second mistake)
then retrospective on both issues - it tried to persist the lesson learnt in the wrong place (there's write gate it ignored) - third mistake
lastly - it promised not to repeat the above mistakes, but it didn't actually persist anything (fourth mistake)

context at max was 64%, so its context was supposed to be healthy.
