Description

I suggest that the app logs failed attempts much like the nextcloud core logs failed logins. That would allow to handle these security issues with external tools like fail2ban.

Research direction

The issue names no file, test, or entry point. Start by locating how failed TOTP attempts are handled in the twofactor_totp provider, then compare that behavior with Nextcloud core's failed-login logging and the needs of external tools such as fail2ban. Done means failed attempts are logged in a usable form, with the expected behavior clarified.
