The iOS client has a functionality to upload the whole phone storage.
That's nice and works fine.
But very often, not all media in the library is not located on the phone but swapped out to the iCloud.
The iPhone or iPad downloads those files on demand, when they are accessed.
To be able to really replace the iCloud with Nextcloud a funtionality is needed to effectively sync the whole library, including media currently swapped out to the iCloud.
This functionality is implementable. Applications like Photosync are doing exactly that.
