Description

Steps to reproduce
  1. create an appointment schedule
  2. in a private browser window or a different browser on your mobile phone, visit the appointment schedule and attempt to schedule an appointment
  3. discover how hard it is to find the submit button.
Expected behavior

The layout of the page should be responsive, so the button to submit the form is visible and easy to find even when the keyboard is open.

Actual behavior

The submit button is hidden off to the right at the top, only accessible after closing the keyboard. Here's a screen recording from my iphone:
